A. Any person desiring to obtain a massage establishment license or manager permit shall make separate application to the City Clerk on the form provided by the City Clerk for that purpose. The City Clerk shall refer the application for each applicant, controlling person and designated agent to the Director of the Police Department, or his designee, for appropriate investigation.

B. The application shall be accompanied by a non-refundable fee of:

C. After approval and prior to the issuance of a massage establishment license, the applicant shall pay a first year license fee of $30.00.

D. In addition to the fee required by subsection B, the applicant, each controlling person and the designated agent for a massage establishment license shall pay the City Clerk the fee established by the Director of the Arizona Department of Public Safety for the processing of state noncriminal justice fingerprints. This fee shall be paid for each person, after the first person, required to submit fingerprints pursuant to Section 10-20 or Section 10-20.1 (Ord. No. G-1117, § 2; Ord. No. G-1564, § 1; Ord.
