a. The Election Board at each voting center shall maintain a list of persons voting. Such list shall be known as the Voting Center Signature Roster. The name of each person who votes must be entered on the Voting Center Signature Roster and be consecutively numbered in the order of voting. A person must sign the roster before being permitted to vote. An Election Board Inspector or Judge may sign the roster for a person who is unable to sign because of physical disability. In such case, the person’s name shall be written with red ink and no attestation or other proof shall be necessary.

b. Each Voting Center Signature Roster shall bear the title "Voting Center Signature Roster", the words "City of Phoenix, Arizona", and shall identify the voting center at which it is used. The Election Board Inspector shall certify in the roster that the signatures, except those in red ink, are true and correct signatures of all persons who voted in the voting center on the date indicated. The Inspector shall further certify that valid proof of identity was received from every voter, or if not, that a provisional ballot was voted due to a lack of valid proof of identity.

c. The Election Board Inspector is charged with the duty of returning the Voting Center Signature Roster to the City Clerk with the unofficial returns of the election.

d. Each Voting Center Signature Roster shall be preserved for the period of time required by law. (Ord. No. G-5390, § 1, 2009; Ord. No. G-5560, § 1, 2010)
