If for any reason, the official ballots are not delivered to a voting center, or if after delivery, they are lost, destroyed, or stolen, the Ballot Judge may substitute in place of the official ballots either sample ballots or paper ballots printed or written in substantially the same form as the official ballots. If substitute ballots are used, The Election Board Inspector shall deliver to the City Clerk a sworn written statement explaining why, and in what form, substitute ballots were issued to voters.
